As a Hospice Client Care Coordinator (CCC)you will be called to care when you’re needed most. As part of Interim HealthCare, you’ll support a full range of patient services to bring comfort and dignity to our clients.

As a Hospice Client Care Coordinator (CCC), you will:   

  • Facilitate new client enrollment with initial and subsequent screening assessments and further maintain client’s healthcare record 
  • Identify resources to address client’s health, daily living, and safety needs  
  • Perform administrative tasks such as scheduling, logging, documenting all appointments and services 
  • Communicate to clients through telephone, computer, and home visits 
  • Coordinate with RN and pharmacy medication set-ups and reminders 
  • Accompany and/or arrange transportation for the client to the provider health care appointments 
  • Be a family liaison and coordinate with the family, designees, and providers 

 

To qualify to be a Hospice Client Care Coordinator (CCC) with us, you will need:  

  • Education: High School graduate or equivalent 
  • Healthcare experience is required
  • Customer service experience is required
  • Experience in working in the senior field is preferred  
  • Must have a valid driver’s license
