The LPN will render nursing care in terms on individualized patient needs in conformance with recognized nursing techniques, procedures and established standards based on the scope of practical nursing.

Essential Duties and ResponsibilitiesIt is expected that all duties and responsibilities of this position will be performed in a manner that reflects the values of Catholic Healthcare Partners, which are Human Dignity, Justice, Compassion, Sacredness of Life, and ServicePerforms patient assessment and care planning, considering the patient’s age group, severity of condition/disease to include physical, psychosocial, cultural, environmental, self care and educational needsUtilize the nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of the patient’s careAssess the patient; collect and document the patient’s current health status and medical historyTeach patient/significant other appropriate health information in a timely manner and share written informational materialCommunicate with the physician regarding patient needs, the nursing assessments, and recommendations on a daily basis, and as neededIdentifies patient/family individualized learning needs and implements measures to meet those needs based on patient/family capabilities to learnPerform direct patient care, utilizing established procedures, policies, and standardsDemonstrate knowledge and understanding of all policies and procedures and ability to reference them from appropriate books and manualsDemonstrate ability to handle emergency situations in a prompt, precise, and professional mannerSchedules tests as directed by providerAssists physician and office procedures and testingImplements care in a safe, knowledgeable, skillful, consistent and continuous mannerEvaluates the effectiveness of equipment and processes for achieving positive patient outcomesAdheres to the safety policies and procedures of SRPSRecognizes and reports potential safety issuesDemonstrates ability to utilize biomedical equipmentMust be trained and competent in the Scheduling, MA/Nurse 100 & 200 functionality of CarePATHMaintains confidentiality and actively supports patient/family rightsSupports and contributes to organizational cost-saving initiativesServes as a patient advocateAdminister medications according to practice policies and proceduresMaintain a clean, neat, and safe environment for patient and staffAttends staff meetings as directed by the practice manager/supervisorAssist patients, family members or other clients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful mannerAnswer and refer telephone calls or other inquires to ensure accurate and timely communications are facilitated, triages phone calls as necessaryDemonstrate sound judgment by taking appropriate actions regarding questionable findings or concernsEnsure compliance with regulatory standardsConsistently demonstrate ability to respond to changing situations in a flexible manner in order to meet current needs, such as reprioritizing work as necessaryMinimize non-productive time and fill slow periods with activities that will enable you to prepare to meet the future needs of SRPS (education, organizing, housekeeping, assisting others) Maintain regular attendance and do not incur excessive overtimePerforms all other duties as assigned Qualifications1 to 3 years experience preferred.
